If an approved volunteer, Independent Contractor, or Steering Committee Member (outside of UNC system) needs UNC Access to perform their duties, you will need to submit an Affiliate Access Form. Please complete the attachment and then upload and submit it below.
PLEASE DO NOT ENTER A SOCIAL SECURITY NUMBER ON THE ATTACHED AFFILIATE FORM.
