The BONUS form is used to submit an EHRA/SHRA Sign-On Bonus Recommendation Form*, EHRA/SHRA Retention Recommendation Form, or an EHRA Performance Bonus Recommendation Form.  Please make sure and review the policies found below prior to submitting a request to our office.  If your request is approved by Provost Business Operations Human Resources Officer and the Office of Human Resources, our office will provide that approval to you along with next steps.

*The Sign-On Bonus verbiage must be included in the position summary prior to posting.
