This form is used for creating a new SHRA permanent position or modifying a current SHRA permanent position. There is a not a “SAVE” feature on our forms. For this reason please complete the Position Description Template (formally SHRA PD-102) prior to completing this form.  Please click here for some helpful instructions for completing the Position Description Template.  Please save 5% for principal functions to read “Other Duties as assigned”.   The Primary Purpose of the Organization (department summary) is already in the system so please only list the position summary.   The required qualifications, competencies, and experience cannot supersede the state minimums.  Please refer to the list of Job Functions when assigning these in the Principal Functions section of the Position Description Template.

If you are requesting a salary increase along with a position modification, you will be asked to attached a Rainbow Form and updated Org Chart (that reflects the modification).  Please do not communicate with your employee any salary increases without receiving approval from our office.
